INSTRUCTIONS

How do I file for my Premium Pay Overtime?

It’s easy, simply:

On an IPHONE:

To Download Premium Pay Form: CLICK HERE

It will open in your browser
Go to the Square-arrow share icon
Select "import to Acrobat" link on bottom of list
Select "import to Acrobat" link at top of options
Fill in the fields that apply to you
Click on the Email link to SD911TER@AMR.net link
The document will open your email and automatically:
- Attach the form to the email
- Address the email for you
- Puts the form name on the subject line
All you have to do is hit SEND!
Close out of Acrobat

On an ANDROID Device:

To Download Premium Pay Form: CLICK HERE

Go to the 3 dots share icon (top right screen)
Select "open with..." link below "send file"
Select "Adobe Acrobat" Icon in options
If first-time, hit the "X" in top right. There is no need to signup
Fill in the fields that apply to you
Click on the Email link to SD911TER@AMR.net link
The document will open your email and automatically:
- Attach the form to the email
- Address the email for you
- Please add C7 form on subject line
All you have to do is hit SEND!
Close out of Acrobat

If you need to send another form, go to your browser and download another copy with the link above.

NEVER EDIT AN OLD FORM TO SUBMIT AGAIN!

It can’t get easier than that! so don’t cheat yourself out of money that is owed you!
